Huron Valley Genetic Consulting can work with you and your staff to provide genetic counseling services to your patients.  Simply complete the one page intake form below and fax it to us at 248-778-4065.  Once we receive the intake form we will contact the patient and set up a convenient appointment for them.  Please click the link below for our intake form.

 

There is also a link below for our family history form. This form is completed by the patient and faxed to HVGC before the appointment.  Your office can print the form for the patient or the patient can access the form here.
